The front free endpaper bears the gift note, "Presented by John Boardman M.D., Nov. 1865." The title page bears the signature, "J. Boardman, 1855."
1856 atlas to Vol. 12 : The volume is half-bound in dark brown morocco with olive cloth over boards, with the spine title stamped in gilt along with a representation of an eagle and the words, "UNITED STATES OF AMERICA."
