The binder's imprint, "Fred Korsmeier, Bookbinder, 1030 Polk St., San Francisco 9, Calif.," is on a sticker in the lower left corner of the back cover verso. The left front endpaper bears an ownership card printed, "William Duncan, M.D., Tyldesley, No., with the pencilled number "1758."
